Divorce/Separation & (Legal) Informal and Formal Child Custody Arrangements

In an effort to maintain and uphold ethical standards regarding “Informed Consent” and services to minors, a consenting signature by both parents is required to initiate treatment.

Four Elements Therapy will not be able to begin treatment until all required signatures are recorded and documented.

 

Divorced: Court documentation that highlights the legal custody arrangements for both parents is required.

 

Separated: Both parental signatures and/or if there is a legal separation, court documents that highlight the legal custody arrangements for both parents is required.

Informal Child Custody Arrangements w/o Family Court Involvement: Both parents must consent to treatment on behalf of the minor in the form of a parental signature.
